Photography Consent

We would like your permission to use before and after photos for advertising. For example, in portfolios, online and in print ads, etc. Your consent is necessary regarding this. Please initial and provide social media handle if you would like to be featured and tagged.

APPOINTMENT: RESTRICTIONS

Candidates seeking permanent make-up treatments who may not be eligible for PMU:   (initial after reading) • Under 18 years of age (even with parental consent)  • Pregnant and/or Nursing women  • People with diabetes (require medical clearance)  • People with glaucoma and people who are taking blood-thinning medication (e.g., aspirin).  Require medical clearance. • People with skin diseases such as psoriasis, eczema and undiagnosed rashes or blisters on the site that is to be treated.  • Epilepsy  • Heart conditions, uncontrolled high blood pressure, poor general health.  • People with allergies to makeup   • History of keloids or hypertrophic scarring  • Those with easily triggered post-inflammatory hyperpigmentation are not good candidates  • Those with transmittable blood conditions like HIV or Hepatitis  • Active skin cancer in the area to be tattooed  • Hemophilia or other healing disorders  • Any treatment, medication, or illness that compromises the immune system or healing, makes you NOT a good candidate for micropigmentation procedures.  • Accutane in the past year  • Let us know if you have/had allergic reactions to topical anesthetics.
